Our commitment

Stillwater is a Health Information Custodian under Ontario's Personal Health Information Protection Act (PHIPA). We collect, use, and disclose personal health information only for the purposes of providing care, with your knowledge and consent except where the law allows or requires otherwise.

What we collect

We collect the information necessary to provide safe, evidence-based care. This includes contact details, health history, the records of your visits, lab results you choose to share with us, and communications between you and the clinic.

How we use it

Your information is used to assess and treat you, to coordinate care between practitioners at the clinic, to communicate with your other healthcare providers when you have authorized us to do so, to bill insurance providers when applicable, and to meet our legal and regulatory obligations.

How we protect it

Records are stored on secure, PHIPA-compliant systems hosted in Canada. Physical records are stored in locked cabinets in restricted areas of the clinic. Staff are trained on privacy and sign confidentiality agreements as a condition of employment.

Your rights

You have the right to access your records, request a correction, withdraw consent for specific uses or disclosures, and ask questions about our privacy practices. To exercise these rights or to raise a concern, contact our Privacy Officer at hello@stillwaterclinic.ca or (416) 555-0184.

If you are not satisfied with our response, you may contact the Information and Privacy Commissioner of Ontario at ipc.on.ca.
